These instructions only apply to apprentice, general, and master falconry permits. Abatement, propagation and rehabilitation permit have different rules and it is up to the individual acquiring these permit to be familiar with the regulations and reporting requirement of the activities. Federal 3-186AS Reporting on USFWS website (database) located here https://epermits.fws.gov/falcP/

The Federal 3-186AS must be completed within 10 days for each migratory bird held under the permit that meets any of the conditions below.
 
– Transfer
– Acquisition/trapping
– Release
– Loss
– Death
– Rebanding
– Microchip

NOTE: The 3-186AS does NOT need to be completed annually unless any of the conditions above are met.
